This is a pilot service being run in one Health Board area in Wales and will run until 1,000 kits have been allocated. The service will then be evaluated to ensure that it is the right thing for Wales so we would be grateful if you would complete the survey after ordering your kit.
If you live in the Hywel Dda University Health Board area you can use this service to get a test kit for Chlamydia and Gonorrhoea that you can use in your own home and then post to the laboratory in the pre-paid envelope.
You will receive your results within two weeks of returning your sample.
In order for us to provide this service we will need some personal data from you which will be collected on a system hosted by Signum Health. We only collect and process the personal data that is necessary for the particular task that we are carrying out. Public Health Wales recognises that your personal data is very valuable, and so we take its security very seriously. We employ robust technical measures to secure your personal data and access to it is restricted to people who have a need to process it in line with their work. In line with the requirements of the General Data Protection Regulations (GDPR) whenever we collect or process personal data from people we must advise them of how and why we process it, what we do with it and who if anyone we share it with. We do this by means of a Privacy Notice.
